For 2009 registration for both Winter Camp and/or the Full Season will be performed on-line and must be completed before equipment can be issued and before a player can attend any practice, indoor or outdoor. Be prepared with 2 copies of ID, contact information and email address, medical information, personal stats and payment method:

If you have already registered with MTW using online registration you have
created an ID and Password. You will need this in order to register.

Proof of ID:
A photo ID, proof of age and proof of Ontario residency as of January 01 of the playing year is required.

a) Driver license
b) Students Card (picture) & school transcript (address and age) **best for peewees
c) Ontario Photo Health Card
d) Hospital Card

other forms of ID could include:
-Birth Certificate
-Baptismal Certificate
-Valid Passport, Citizenship Card
-Social Security Card
-Credit Card
-Bank Card
-Landed Immigrant Certificate
