ALL PLAYERS/PARENTS MUST ENROLL ONLINE  AND SUBMIT FORMS AND FEES IN PERSON
 1. Complete On-Line enrollment form and submit
2. Print out Enrollment Confirmation Form after form is submitted
3. Download and complete medical and registration forms.
4. Photocopy 2 forms of ID. One with photo and one with address as of April 1 playing year.
