Two (2) pieces of ID
including 1 photo ID and one with proof of age
and proof of Ontario residency as of April 1st of the playing year is
required.

b) Students Card
(picture) & school transcript (address and age) **best for peewees
c) Ontario Photo Health Card
d) Hospital Card
other forms of ID could include:
-Birth Certificate
-Baptismal Certificate
-Valid Passport, Citizenship Card
